Insurance Verification
Should you need to determine a specific patient’s coverage for the GRAFTJACKET® Regenerative Tissue Matrix - Ulcer Repair Product, our reimbursement counselors will contact the specific payer to determine:

• Coverage for the specific GRAFTJACKET® Matrix Product, including prior authorization requirements.
• Cost-sharing responsibilities for the GRAFTJACKET® Matrix Product.
• Network or site of service restrictions for obtaining the GRAFTJACKET® Matrix Product.

Assistance to Accounts for Prior Authorization
Reimbursement counselors will:

• Research prior authorization requirements for the GRAFTJACKET® Matrix Product.
• Initiate the prior authorization process (as allowed by payer).
• Provide template letter of medical necessity.

Billing/Coding Assistance
Our counselors are available to assist with questions related to billing and coding for the GRAFTJACKET® Matrix Product and the procedures that are associated with use of these products.

Claims Assistance
Our counselors can assist with various points of the claim handling and management process. We are available to review completed claims for accuracy (paper claims only) or help with guidance in completing and submitting electronic claims.

Claim Denial/Appeal Assistance
In the event that you receive a denial for the GRAFTJACKET® Matrix Product, our reimbursement counselors will contact the payer and:

• Determine the appropriateness of that denial.
• Confirm the reason for the denial.
• Determine the process and requirements for submission of the appeal.
• Provide template letters of appeal.

Research Product Coverage
We are also available to help with more general research of coverage for specific payers. Research of product coverage may include research of coverage policies or allowed amounts for use of the GRAFTJACKET® Matrix Product.
